Form Description
Active Duty Foreign Military Waiver Request for Non-US Active Duty Military to receive in-state tuition.
Adjusted Credit Option Form Request for the one-time option of having a NMSU permanent academic record adjusted.
Application and Certification for Establishing Credit by Examination Request to receive credit for a course that the student feels proficient in, with required approvals.
Catalog Course Description Request Form Request for students who need to have the course descriptions of the courses that had completed at NMSU in order to provide these to individuals that may request them (i.e., other institutions for evaluation purposes, employers, etc.)
Change of Schedule Form Form to administer a schedule change (Formerly the “Add/Drop Slip”).
Demographic Change Form Form to request an update of Legal Name, Social Security Number, or Gender.
No Release of Information Objection of currently enrolled students to release of directory information to ANY public entity or being.
Official Withdrawal From NMSU Form Official request to be withdrawn from any NMSU campus for the current semester.
Petition for NM Resident Tuition Classification Application packet to petition for NM resident tuition.
Petition for Medical Withdrawal Request to withdraw based on medical conditions for student or immediate family member of a student.
Petition for Retroactive Registration Change Request to retroactively make changes to a student’s registration after the published deadline date.
Resident Tuition Application for Active Military, Veterans, and Dependents of the US Armed Forces Request for Active Duty US Armed Forces, Reserves, New Mexico National Guard, Veterans and Dependents to receive in-state tuition.
Senior Citizen Waiver Request for the NM Senior Citizen Reduced Tuition Program.
Solomon Act Request Official military branches request to obtain student directory information.

For Faculty and Staff

Form Description
Academic Program Transition from Face-to-face to Distance Education Request Departmental/College request for a currently existing academic program to transition from a face-to-face instruction to a distance education instruction.
Academic Schedule Modification Request Request for NMSU main campus college or department representatives to add, drop, or change courses.
Course Action Form- CAFS A system for faculty/staff to submit changes to existing courses, add new courses and delete old courses for the entire NMSU System. Users must have access in the system in order to submit CAFs but all faculty/staff have view access. To gain access contact the Catalog Editor within the Registrar’s Office.
Curriculum Change Form – Applied Associate Major and/or Degree
This form is used when a curriculum change is needed for a change to either (or both) an Applied Associate Degree or an Applied Associate Major.
Curriculum Change Form – Associate Major and/or Degree
This form is used when a curriculum change is needed for a change to either (or both) an Associate Degree or an Associate Major.
Curriculum Change Form – Bachelor’s Major and/or Degree
This form is used when a curriculum change is needed for a change to either (or both) a Bachelor’s Degree or a Bachelor’s
Major.
Curriculum Change Form – Master’s Major and/or Degree
This form is used when a curriculum change is needed for a change to either (or both) a Master’s Degree or a Master’s Major.
Curriculum Change Form – Doctoral Major and/or Degree
This form is used when a curriculum change is needed for a change to either (or both) a Doctoral Degree or a Doctoral Major.
Curriculum Change Form – Certificate (Graduate)
This form is used when a curriculum change is needed for a change to a Graduate Certificate.
Curriculum Change Form – Certificate (Community College)
This form is used when a curriculum change is needed for a change to a Certificate at one of the Community College Campuses.
Curriculum Change Form – Concentration/Minor (Undergraduate/Graduate)
This form is used when a curriculum change is needed for a change to either an Undergraduate/Graduate Concentration or an Undergraduate/Graduate Minor.
Curriculum Inactivation Form – Community College Campus
This form is used when a certificate, degree and/or major is being inactivated at one of the Community College campuses. This
form is used whether there is enrollment/staff-faculty lines with the program or not, however the notification and attachments are slightly different if there is enrollment/staff-faculty lines.
Curriculum Inactivation Form – Las Cruces Campus
This form is used when a certificate, degree and/or major is being inactivated at either the Undergraduate or Graduate within the Las Cruces Campus. This form is used whether there is enrollment/staff-faculty lines with the program or not, however the
notification and attachments are slightly different if there is enrollment/staff-faculty lines.
“I” Grade Information Form
This form is used when instructors are issuing an “I” grade and indicates all actions required by the student in order to have the “I” grade removed.
New Curriculum Form – Applied Associate Major and/or Degree This form is used when a new Applied Associate Degree, an Applied Associate Major or both is needed. This process is more extensive than a change form and requires external approval from HED and HLC before being processed in banner and added to the catalog/degree audit systems.
New Curriculum Form – Associate Major and/or Degree This form is used when a new Associate Degree, an Associate Major or both is needed. This process is more extensive than a change form and requires external approval from HED and HLC before being processed in banner and added to the catalog/degree audit systems.
New Curriculum Form – Bachelor’s Major and/or Degree This form is used when a new Bachelor’s Degree, a Bachelor’s Major or both is needed. This process is more extensive than a change form and requires external approval from HED and HLC before being processed in banner and added to the catalog/degree audit systems.
New Curriculum Form – Master’s/Doctoral Major and/or Degree This form is used when a new Master’s/Doctoral Degree, a Master’s/Doctoral Major or both for each level is needed. This process is more extensive than a change form and requires external approval from HED and HLC before being processed in banner and added to the catalog/degree audit systems.
New Curriculum Form – Certificate (Graduate) This form is used when a new Graduate Certificate is needed. This process is more extensive than a change form and requires external approval from HED and HLC before being processed in banner and added to the catalog/degree audit systems
New Curriculum Form – Certificate (Community College) This form is used when a new Certificate at one of the Community College Campuses is needed. Certificates can be either a Certificate of Achievement or a Certificate of Completion, they can also be embedded in an Associate’s Degree or not.
New Curriculum Form – Concentration (Undergraduate/Graduate) This form is used when a new Undergraduate and/or Graduate Concentration is needed. Please make sure to indicate if both a Concentration at the undergraduate level and the graduate level are being submitted for approval. Concentrations are Transcripted for students, as long as the student has the concentration on their academic record. If the concentration is in banner than it must be labeled as a “Concentration” in the catalog.
New Curriculum Form – Minor (Undergraduate/Graduate) This form is used when a new Undergraduate and/or Graduate Minor is needed. Please make sure to indicate if both a Minor at the undergraduate level and the graduate level are being submitted for approval.
Out of Cycle Degree Audit Change Request for an out of cycle degree audit change.
Student Absence/Lack of Progress Report This form is used for instructor’s to request to have a student administratively withdrawn from a course due to lack of attendance or progress.
