Change of Class Schedule Form | Student request for schedule change, with special permissions or after deadline to add, drop and withdrawal through myNMSU. |
Credit by Examination Application | Request opportunity to demonstrate proficiency, by examination, to receive credit for a specific course. |
Demographic Information Change Form | Request to change your legal name, social security number, and/or gender. |
Military – In-State Tuition Waiver for Active Duty, Veterans, and Dependents of the US Armed Forces | Request for members of the Active Duty US Armed Forces, Reserves, New Mexico National Guard, Veterans and their Dependents to receive in-state tuition. |
Military – In-State Tuition Waiver for Active Duty Members of Foreign Forces | Request for Non-US Active Duty Military members to receive in-state tuition. |
NM In-State Resident Tuition Rate Petition | Application to petition for in-state tuition rate. |
No Release of Information Request | Objection to release of directory information to ANY public entity or being, except as required by law. |
Medical Withdrawal Petition | Request to withdraw from all classes for the semester based on medical condition of the student or an immediate family member of the student. |
Registration Deadline Waiver Petition (Retro) | Request to change a student’s registration after the published deadline date for the semester / part of term. |
Senior Citizen Tuition Rate Petition | Application for the State of NM Senior Citizen Reduced Tuition Program. |
Solomon Act Request for Information | Request, by official military branches, to obtain student directory information. |

“I” Grade Information Form | Agreement, between instructor and student, that indicates actions required by the student in order to have the “I” grade removed. Must be completed, and signed by all parties, when an instructor is issuing an “I” grade. |
Out of Cycle Degree Audit Change Request | Departmental request for an out of cycle degree audit change. |
Student Absence/Lack of Progress Report (Administrative Withdrawal) | Instructor request to have a student administratively withdrawn from a course due to lack of attendance or progress. |
